The Supports Coordinator (SC) helps to locate, coordinate, and monitor services and supports for individuals on their caseload. They work with families and teams to assist folks in having an Everyday life and enjoying their homes and communities. The population served have intellectual/developmental disabilities and/or Autism. The SC develops an Individual Support Plan (ISP) for each person and their team/family, and reviews this plan on an ongoing basis to ensure each person has the supports they need at each stage of their lives. They encourage individuals to work in their communities if appropriate. Initial SC Training is required, and ongoing training occurs through the supervisor and shadowing with peers. The SC position is a blend of time in the office and data entry, along with traveling to meet in person with individuals and teams. It is multi-faceted, interesting, challenging and rewarding.
