Wyckoff Heights Medical Center is looking for a Director of Virtual Primary Care for Telehealth services to join the Department of Medicine. The candidate will use  digital information and communication technologies to provide health care services remotely and manage the health care of our patients. The ideal candidate will provide Primary and Family evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients. The candidate will have administrative responsibilities that will ensure that the primary care virtual program meets the expectation of the patients and Wyckoff.  Will collaborate with leadership and providers to provide evidence based practice to the virtual primary care program. They would provide excellent patient service skills and is committed to the highest standards of patient care. 

The candidate will ensure that the virtual primary care program meets the needs of the patient by ensuring that patients have access to telehealth services provided by Wyckoff and  work with leaders to address any barriers to patient care.

  • Obtain and document patient history as appropriate to chief complaint or reason for visit.
  • Conduct relevant  exams and evaluate patients, documenting pertinent positive and negative findings.
  • Diagnose and treat disease and manage chronic illness.
  • Prescribe medication in accordance with New York State and federal law.
  • Order appropriate laboratory, radiological and special examinations or tests; interpret results.
  • Provide preventative health services, including health education and counseling.
  • Collaborate with other clinicians and staff members to ensure continuity of care of patients.
  • Perform other job-related duties as required. Attend staff educational programs and meetings.
  • Develop and implement individualized care plans for patients.
  • Maintain accurate and up-to-date medical documentation.

Requirements/Qualifications:

  •  Master of Science degree in Nursing—Adult-Gerontology Primary Care Nurse Practitioner (preferred) or Family Nurse Practitioner, Doctorate of Nursing Practice preferred
  • Graduate of a Nurse Practitioner education program registered by the New York State Education Department.
  • Certification as an Adult-Gerontology Primary Nurse Practitioner or Family Nurse Practitioner from a New York State approved accrediting body – American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP) 
  • Must possess current, unrestricted New York State Registered Nurse license and Nurse Practitioner license
  • Previous experience working as a Registered Nurse in acute care setting, preferred, for at least four years
  • Telemedicine or virtual care experience a plus
  • Seeking both experienced (2 years and above) and newly trained providers
  • Must possess active Drug Enforcement Agency (DEA) registration or be eligible for registration
  • Must possess active Medicare/Medicaid Enrollment or be eligible to enroll
  • BLS or ACLS certification required
  • Must possess strong communication skills both oral and written
  • Comfortable with Electronic Medical Record (EMR) system and Microsoft Office
  • Bilingual Spanish (preferred)
